Sagepoint Home Care

Sagepoint Home Care provides assistance with daily living activities allowing more older adults to maintain their independence in the comforts of their own home. Our home care services include Charles, Calvert, and St. Mary’s Counties.

Companion Care For Your Loved One.

Personal Care For Your Loved One.

Our personal care goes beyond everyday chores. Personal Care is performed by a Certified Nursing Assistant (CNA) and includes assistance with the following:

  • Needs-Assessment by Nurse (R.N.) Case Manager.
  • Stand-By Supervision of Daily Activities.
  • Preparation/ Serving of Meals Based on Needs.
  • Feeding Assistance.
  • Medication Reminders.
  • Bathing/Showering.
  • Grooming/ Dressing.
  • Hair, Skin, and Oral Care.
  • Toileting Assistance.
  • Ambulation Assistance.
  • Turn and Position Transfers.

Respite Care. Support For You.

Caregiving can be challenging. Whether you’d like a few hours to do some shopping or go fishing, or take the kids on vacation, we will assist your loved one in their own home while you are gone. The services offered through our respite care are detailed above under companion care and personal care. Please contact us today to discuss how we can help you take a little time for yourself. In some cases, you may be eligible to receive financial assistance via Respite Grant programs offered through your local Department of Aging.

Adult Day Services

Rehabilitation Care

  • Highly trained, experienced staff of RNs and LPNs oversee all nursing care.
  • High staff-to-patient ratio means more individual attention for patients.
  • Staff with specialized training in: Orthopedic Rehabilitation, Restorative Care, Wound Care, Infection Control, Pain Management.
